THE STATE OF UTAH TO: GORDON ARDELL BARNES

	You are hereby summoned and required to file an answer in writing to
the attached Complaint with the Clerk of the above-entitled Court,
located at 2525 Grant Avenue, Ogden, Utah 84401, and to serve upon or
mail to Parr Brown Gee & Loveless, c/o Royce B. Covington, counsel for
the above-captioned Plaintiff, 185 South State Street, Suite 800, Salt
Lake City, Utah 84111, a copy of the Answer within twenty (20) days
after service of this Summons upon you.

	If you fail to do so, judgment by default will be taken against you
for the relief demanded in the Complaint, which has been filed with
the Clerk of the Court, Second Judicial District Court, Weber County,
at 2525 Grant Avenue, Ogden, Utah 84401, a copy of which is annexed
and herewith served upon you.
