Helper Medical Clinic is soliciting proposals to provide interior
remodeling to their existing building located at 125 South Main
Street, Helper, Utah as defined by the Contract Documents.

Bidding Documents are included within the project manual and drawings
dated September 16, 2009 prepared by the Engineer.

This project is funded iwth a grant made available by The American
Recovery and Reinvestment Act. The Contractor will be required to
comply wiht all federal labor standards and attendant laws, including
the payment of the most current Davis-Bacon wages and use of products
manufactured in the United States as indicated within the Summary of
Work.

One set of printed Bidding Documents per general contractor, will be
available at EIS (Engineering Company), no sooner than December 8,
2010. The contractor will be charged $100 per set.

In order to be considered responsive, sealed bids are to be submitted
on the bid form copied from the Project Manual and shall be submitted
to EIS no later than 4:00 p.m. January 7, 2011. Please note that time
os of the essence for this contract. Bids will be evaluated based on a
combination of hte proposal amount and the number of days proposed to
substantially complete the Work.

Bids shall be submitted in a sealed envelope as stipulated in the
Instruction to Bidders. Bids shall be sent via U.S. Mail, commercial
carrier, or hand delivered. Bids will not be accepted by facsimile.
Bids received after the time stipulated for receipt of bids will be
returned unopened.

A pre-bid conference will be held at the project site at the time and
date indicated within the Instruction to Bidders.

The project will require a Performance Bond, Payment Bond, Liability
Insurance, Worker's Compensation Insurance and current Utah State
contractor license.

The Owner reserves the right to reject any or all proposals and waive
any informality or technicalitty in any proposal in the interest of
the Owner.
