Request for Proposal

    
Emery County School District is now accepting proposals to replace
approximately 30,000 square feet of Carlisle or equivalent EPDM
Ballasted roofing at Canyon View  Junior High and  approximately
30,000 square feet at San Rafael  Junior High school in Emery County,
Utah.

 A Mandatory Walk-Through will be held on Tuesday April 26, 2011
starting at Canyon View Junior High School at 9 a.m.  Must Field
Verify All Measurements.

 Bid Opening will be Tuesday, May 3, 2011 at 1 p.m. at the District
Office at 130 North Main in Huntington, Utah.  Proposals must be
received on or before Noon on Tuesday, May 3, 2011.  

 All Contractors shall provide documentation prior to the execution of
the Agreement, that Contractor is registered and participates in the
Status Verification System (the electronic verification system
operated by the Federal Government known as E Verify) to verify the
work eligibility status of the Contractor and/or employees.  In
addition, the selected contractor shall also obtain affidavit
certification from all engineering firms and other subcontractors who
are utilized on this project(s) that they have verified the work
eligibility status of their respective employees through the Status
Verification System (E-Verify) as required by Utah Code Ann.,
§63G-11-103.

 Proposals may be mailed or faxed to Kerry Lake, Building Construction
Supervisor.  Contractors must submit a Bid Bond in the amount of five
percent (5%) of the bid amount, made payable to the Emery School
District.  A Bid Bond must accompany each bid.  The Emery School
District reserves the right to reject any or all submittals/bids or to
waive any informality or technicality in any submittal/bid in the
interest of the School District.  Owner reserves the right to accept
or reject the lowest responsible submittal/proposal.
