Carpet Cleaning

	Proposals must be in a sealed envelope clearly marked Request for
Proposal - Contracted Carpet Cleaning and delivered or mailed to:

by June 7, 2013 at the close of business.  Questions regarding this
proposal should be directed to Jeff Horrocks, Commission Chairman at
(435) 749-9956. 

	The purpose of this RFP is to solicit responses from qualified,
licensed, businesses to furnish Carpet Cleaning Services on an
as-needed basis for the Emery County Courthouse, Public Safety Center,
and other county owned  facilities as needed.  The term of the RFP
will be two years.  Include in the proposal an overview of your
business, years of service,  two references,  and provide a cost per
square foot and clearly specify any additional expenses that will be
charged in connection with this proposal.  The cost per square foot
shall include all materials, equipment and labor.  Work to be
performed under this contract will be during hours when the offices
are not occupied, or at the discretion of the commission chairman. 
Contractor will carry liability insurance.  Payment will be made upon
invoice and county policy.

	Proposal by telephone or facsimile-email will not be accepted.
